The patient portal is your secure online tool to access your health information, request appointments and refills, or communicate with your care team.

Attention Registered Users: The Patient Portal has been upgraded. Your Login and Password from the old portal can be used to login. If you do NOT know your password to login you will NOT be able to do a password reset.  You will need to contact the office to have them issue you an account reset email.

 

Once logged in, you will be asked to change your password and update your security questions and answers. If you have difficulty logging in or don't remember your password, please contact our office at: (541)-930-7255.
